Please fill out the referee evaluation form following the guidelines below:

• Only the team coach or team captain should fill out the form.
• Please wait 24 hours after your game before filling out and submitting a report.
• Specify what rules were broken and eliminate generalizations.
• Don’t use this form as a place to vent. It is meant to be a productive tool to improve the league experience.
